The Florida Technology Council is excited to welcome Unite Us to our membership. Founded in 2013, Unite Us is a technology company that builds coordinated care networks of health and human service providers to address all determinants of health. Originally created to serve veterans, this top 100 healthcare technology organization has grown into a national movement - connecting everyone to the care they need.
How it works
Unite Florida is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs and improve health across communities.
Say Anna, a senior citizen from South Florida requires assistance from her local healthcare provider, Jamie. Jamie carefully screens Anna and identifies her health and social needs. Jamie then uses Unite Us to gain digital consent and electronically refer Anna to multiple community partners. Through the platform, she can seamlessly communicate with the other providers in real time and securely share Anna’s information. As Anna receives the care she needs, every referral, interaction, and outcome is tracked by Unite Us.
Creating a better, safer, stronger, and more efficient Florida
The Herald-Tribune reports that, since coming to the Sarasota area, Unite Us has helped to link traditional health care with an array of social services such as prenatal care, education, and food and nourishment services.
In Florida, Unite Us has a live accountable coordinated care network in 24 counties representing 50% of the Florida population, with 30 more counties going live in the beginning of November. Unite Us will be statewide by early next year, ensuring individuals in need across Florida will be able to get connected to care quickly, efficiently, and compassionately.
Unite Us believes that FTC membership affords the opportunity to accelerate community impact through innovative partnerships, shared expertise, ongoing education, and though leadership that will truly make a difference in the lives of Floridians.
Based in St. Petersburg, Megan Diehl – Unite Us’ Florida State Director, has over a decade of experience working to improve children’s health through program development and leadership at the Alliance for a Healthier Generation, and is also experienced in healthcare marketing, communications, and medical education in the managed care and pharmaceutical sectors.
